Co-Creator Submission Form -The Illinois History and Lincoln Collections – Collecting COVID-19 Stories in Illinois
The Illinois History and Lincoln Collections (IHLC) unit of the University of Illinois Library is collecting evidence of Illinoisans’ experiences with COVID-19. This form is for co-creators when two or more people have contributed to donated materials, or if the individual submitting materials has interviewed anyone else in the course of their documentation. We require the permission of all co-creators of any submissions. Only one person needs to submit the file and complete the full submission form, but everyone who appears in the documentation or contributed to its creation needs to give their permission. This form is for co-creators to grant permission after the materials have already been submitted by another. Please contact the IHLC (ihlc@library.illinois.edu) with any questions.
