Juniors and Seniors in the Minor – PLEASE READ

Juniors and Seniors in the minor – there are still many of you who have not completed your required paperwork in order to participate in clinical experiences!  I have listed the information sent by the OCE recently and I’m also reminding you about the BBP, CBC and Safety Training!  It is very important that you complete these requirements in a timely fashion.  Remember that you are no longer only Illinois students, you are now in an Illinois State Board of Education certification program and will need to follow those procedures as well. If you have not completed all of your requirements, please read on.  If you have, then you need not worry.

The Office of Clinical Experiences requires the completion of two forms at the beginning of the new academic year. In the past, these forms have been paper hard copy, but starting this year, they are electronic.

One form is the Professional Records Statement. This statement informs you that your clinical experiences file may be shared with personnel from school districts in which you are placed for early field experiences and student teaching. Your file may include time reports, evaluations, and various other materials revolving around your placements in the field. Please read the statement, check the Yes box, and submit. The next form is the Research Permission Form. This permission form allows you to grant or decline permission for the Department of Curriculum and Instruction to include some or all of the listed works to be used in research efforts. These forms will only take a few minutes to complete and you will receive an email confirming your submission.

I would like to remind you that these two forms are required and the deadline for submission is Monday, September 10th by 5pm. If we have not received your response by the deadline, then OCE will suspend your placement until we receive confirmation of submission.
