The Program is funded through an Illinois State Board of Education (ISBE) grant that seeks to provide out-of-school time opportunities for youth and allows for up to 100 middle school and 80 elementary school participants, with youth enrolled in the free and reduced lunch program receiving first priority.
Registration is a four-step process, it must be completed in its entirety in one sitting, and will take approximately 30-minutes per participant. If registration is begun and exited prior to full completion (i.e. completion of all four steps), it cannot be resumed, and must be re-started from the beginning.
PROGRAM REGISTRATION IS A FOUR-STEP PROCESS:
(1) Participant information survey.
(2) Parent/guardian information survey.
(3) Review and approval of research consent paperwork.
(4) Completion and receipt of written documents.
INFORMATION NEEDED WHEN REGISTERING INCLUDES:
– Participant personal information (e.g. name, address, date of birth).
– Participant Unit 4 school information (e.g. grade level, school attended, student ID number, free and reduced lunch status).
– Participant transportation needs (e.g. distance to Program, pick up location, drop off location).
– Parent/guardian and emergency contact personal information.
– Participant medical information (e.g. potential conditions, health insurance policy information).
MIDDLE SCHOOL PROGRAM
DATE: Monday – Friday, June 1st – June 25th, 2021.
PARTICIPANTS: Rising sixth to eighth-graders in Champaign Unit 4.
REGISTRATION: Registration is currently closed.
ELEMENTARY SCHOOL PROGRAM
DATE: Monday – Friday, July 5th – July 23rd, 2021.
PARTICIPANTS: Rising first to fifth-graders in Champaign Unit 4.
REGISTRATION: Registration is currently closed.
The Meal Form (as outlined in registration step 4 above) only needs to be completed if the participant has specific dietary restrictions or food allergies and requires a physician’s signature. A copy of the form can be found here: MEAL FORM.
Once completed, the Meal Form should be mailed back to:

Once all registration materials have been completed and returned (i.e. the meal form, if necessary), the participants’ eligibility for the Program, based on grade level and free and reduced lunch status, will be verified in collaboration with Champaign Unit 4 Schools.
Participants who have completed the four registration steps, and meet eligibility requirements, will either be placed into the Program or on the wait list. Verification of eligibility and assignment to the Program or wait list will be E-Mailed to the parent/guardian using that entered in the participant information survey.
