If your group has more than one safety officer (e.g. LSOs for different locations or chemsafety vs. biosafety), please only fill out this form for the single LSO position that is changing. Please provide any additional information in the comments box.
Please note, the previous LSO and the new LSO should plan to attend AT LEAST one LSO meeting together.
